Tiffany Milby is the owner of Acacia Transitions, bringing more than 30 years of real estate experience and a deep commitment to serving seniors and their families throughout the Valley. Dementia Care Certified and known for her calm, personalized approach, Tiffany helps guide clients through the home-selling and transition process with clarity, compassion, and confidence.

Having worked closely with Acacia Transitions for years, Tiffany is honored to carry forward the company’s mission of integrity, trust, and personalized care. As owner, she is focused on preserving the values that built Acacia Transitions while thoughtfully expanding its ability to support seniors during one of life’s most important transitions.

As a licensed practical nurse working in geriatrics for many years, Deborah provides a valuable insight to the needs of our seniors, her compassion emanates in her everyday quest to making seniors lives better. Deborah's experience in clinical rehabilitation and marketing, provides special skills utilized in her position as Community Relations Director.

On the personal side, Deborah enjoys 10 years as a Reiki Master/Teacher and her passion is volunteering with animals, such as the Salt River Wild Horse Management Group and others.

Donald R. Smith is the founder of Acacia Transitions, LLC, established to provide compassionate, full-service support for seniors transitioning from home-ownership to independent or assisted living communities. After more than ten years of dedicated service to seniors throughout the Valley, Don has transitioned ownership of Acacia Transitions while remaining actively involved in an advisory role.

As Founder and Advisor, Don continues to support the company’s mission by offering strategic guidance, sharing his deep community relationships, and ensuring the values and standards that built Acacia Transitions remain at the core of its work.
